When Acupuncture Didn't Work: What Fascial Counterstrain Does Differently

Most of the people who find their way to my office have already tried something. Often several somethings. Physical therapy. Chiropractic. Massage. Sometimes acupuncture, occasionally acupuncture with me.

They got somewhere. Then they stalled.

That plateau is the most common conversation I have. Not "nothing has helped," but "everything helped a little and nothing finished the job." For a long stretch of my career I didn't have a good answer for it. Now I usually do, and it comes down to a technique most people in Michigan have never heard of.

A 22-Year-Old Who Should Have Been Better

He came in a year after breaking his ankle. Clean break, both bones, screws to hold it together. The surgery went fine. The bone healed. By every measure his doctors had, he was recovered.

He wasn't.

The pain in his shin and ankle was close to unbearable. Numbness in places. Burning in others. He'd seen the surgeon, the follow-up doctors, and worked through physical therapy — the whole standard sequence, done properly.

What got me was how much he'd lost. This was a kid who had been active before the break, and a year later he was still on the sidelines, watching. He was sad about it in a way that had nothing to do with the pain score. Twenty-two years old and already resigned to the idea that this was just how his leg was going to be.

We started with acupuncture and myofascial therapy. Both helped. The pain came down, the tissue loosened, he had better days. But it kept coming back, and I could tell we were working around something rather than at it.

The piece that finally moved it was Fascial Counterstrain — specifically the lymphatic and arterial systems.

That's the part worth paying attention to. Not the needles, not the manual work on the muscle. The restriction driving his pain wasn't in the muscle at all. It was in the lymphatic and vascular tissue around the injury — tissue that had been compressed and inflamed since the trauma, that no imaging had looked at, and that nothing in his year of treatment had touched.

A few weeks after those treatments, the pain was gone.

His mother called me afterward. She wanted to thank me, and she was emotional about it — not about the ankle, about having her son back to himself. That call has stayed with me. When someone is 22 and in pain that long, it isn't only a leg problem anymore.

What Fascial Counterstrain Actually Is

Fascial Counterstrain (FCS) is a manual therapy developed by Brian Tuckey, built on the osteopathic strain-counterstrain work of Lawrence Jones.

The premise: when tissue is injured, inflamed, or traumatized, it goes into a protective spasm. That spasm is supposed to be temporary. Frequently it isn't. The tissue stays guarded long after the original injury has healed, and that guarding becomes its own source of pain — sometimes for years, sometimes nowhere near where the problem started.

FCS finds those spasms by palpating for specific tender points, then shortens the tissue into a position of ease and holds it. No forcing, no stretching, no deep pressure. The tissue is given the conditions to release, and it does.

If you've had deep tissue massage, this is the opposite. Patients are often surprised by how little is happening — right up until something lets go.

Why It Reaches What Other Approaches Don't

Most manual therapy addresses muscle. Some addresses the fascia around muscle. FCS goes considerably further than that.

The technique is organized into systems, each targeting different tissue:

Musculoskeletal (MSK-C) — the muscles, joints, and connective tissue people usually picture

Visceral — the fascia surrounding organs, which refers pain to the back, chest, and abdomen more often than most people realize

Lymphatic — the vessels that clear inflammation and fluid. When these are restricted, inflammation sits in tissue and stays there

Arterial — the vessels supplying blood. Restriction here can drive pain, numbness, burning, and cold sensations that don't follow a normal nerve pattern

That last pair is what resolved the ankle case. And it's why "I already tried manual therapy" doesn't tell me much — it depends entirely on which tissue was worked. If the problem lives in the lymphatic or arterial system, a year of excellent muscle work won't reach it.

Who This Tends to Help

In my experience, FCS tends to be most useful for people who fit one of these patterns:

  • Pain after an injury has technically healed — the fracture united, the surgery succeeded, the imaging is clean, and it still hurts

  • Numbness, burning, or tingling that doesn't follow a clean nerve distribution

  • Symptoms with a normal workup — dizziness, ear pressure, headaches, or pain where every test comes back unremarkable

  • Post-surgical pain at or near the site, months or years afterward

  • Anything that responds to treatment and then returns on a predictable cycle

That last one matters. If you feel better for three days after every session and then slide back, something is being managed rather than resolved. That pattern is usually a signal that the actual restriction hasn't been found yet.

What to Expect

FCS is gentle. There's no cracking, no forceful stretching, no working through pain. I locate a tender point, position the tissue, and hold.

Some people feel a change during the session. Others notice it over the following days. Long-standing problems generally take several visits — the ankle case took multiple sessions before it held.

I want to be straight about this: FCS is not a guarantee, and I don't treat it as one. Some people respond quickly, some gradually, some not much at all. The 22-year-old's outcome was his — it isn't a promise I can make to the next person. What I can say is that it reaches tissue other approaches don't, which makes it worth trying when other approaches have run out.

I also don't use it in isolation. Most patients here get some combination of acupuncture, myofascial therapy, and FCS, because those tools do different jobs. Acupuncture calms the nervous system and reduces inflammation. Myofascial therapy releases mechanical restriction. FCS finds what the other two can't reach.

Where to Find Fascial Counterstrain Near Detroit

There aren't many practitioners trained in FCS in Michigan, and fewer still who combine it with acupuncture.

I've completed training in the Foundations, Visceral, and MSK-C systems, and I've continued into the Arterial and Lymphatic systems — the two that resolved the case above. It's ongoing coursework, not a weekend certificate, and I keep at it because it keeps changing what I'm able to help with.
